We offer three types of services:
- Office visits face
to face
- Home visits face to face
- Telephone consultations

Individual Consults: Office Visits, face to
face
Frequency of visits
Determined at the first session after an assessment
of your needs. Customarily for weight loss
and other medical conditions, the first
month you may need more frequent sessions. After
the first month its frequency is discussed and determined.
The length of the follow-up visits vary from 45-60 minutes
Costs
Cost of visits is based on 15 minute units of time
for face to face consults for plans covered
by third party payers.
Ask about our special rates for services not covered by health care insurance
plans.
Face to Face:
Up to 30 minutes
Up to 45 minutes
Up to 60 minutes
Up to 75-90 minutes
Length of visits (usual there may be exceptions)
Visit
1: 60-90 minutes face to face
Visit 2: 45-90 minutes face to face
Visit 3: 30-60 minutes: wear loose clothes; exercise
demo
Thereafter: 30- 75 minutes depending on frequency
Fees and Payment Policies
Payment is due at the time of the consult by cash, check,
or credit card. If you have an insurance
plan that we accept, you may qualify to pay only a co-pay
or discount.
We offer payments
online under Products. Gift
certificates are also available.
Group or Self Pay Contract
Pay at each visit. No billing accepted. Reduced rates may
apply.

Visits and payment for minors
Prepayment
is required if the parent responsible is not present. All
minors must be accompanied by an adult.

Cancellation Policy
We require 24 hours notice for all cancellations. Patients
who do not cancel a full 24 hours before their appointment
will be subject to a $50.00 appointment cancellation fee.
Supplements
We offer an array of natural vitamin minerals supplements
that may be of benefit, but we always prefer food for
nutrients. Ask about our weight loss meal replacements.
Labs and Tests and EKG
If you have a medical condition, we will need the pertinent
medical information from your physician.
Other Health Care provider information
Physical exam
results from your primary physician including abnormal
lab tests results, blood pressure and any other
pertinent information.
Communication with your other health care providers as needed.

Family Nutrition (4 family members)

Tele-consults
No insurance accepted.
Fees based on time: Up to 15 minutes is $30; up to 30
minutes is $60.00.
